IMPORTANT: Once you submit your details to us you will receive an auto email with instructions to complete the ACC access form. You can also download the ACC access form here.

The ACC access form gives us authority to access your ACC account/s as well as enable us to act on your behalf and have visibility to your ACC information.

Important things to note when completing the access form:

Section 1a - the following sections must be completed

  • Your ACC account name
  • ACC or IRD number
  • The postal address ACC have on record

Section 1b
If you are completing an ACC access form for a company, please provide your name and position.

Section 4

Please be sure to sign and date.

Your completed form can be emailed to hello@accassist.kiwi we endeavour to respond in person within 24 hours.
